Telephone Referrals
Our scheduling personnel will be happy to make appointment requests via telephone.  You will be asked to fax the following patient information at the time that the appointment is made.

  - Recent cardiac testing and procedures
  - Pertinent patient reports and EKGs
  - Copy of patient's insurance cards
  - Appropriate referrals (if required)

Fax Referrals
You may also fax over your request by printing the appropriate form below.  Please complete it in its entirety and fax it along with the appropriate patient records to our office.  We will then contact the patient and schedule the appointment.  Once the appointment is scheduled, the form will be faxed back to your office with the top right-hand corner completed.  This will be your verification that the appointment has been made.
